Full Name / Alias: Include your full name as well as any nicknames.
Gender: Male or female?
Age: How old you are. Your real age should be about ten-twenty times how old you look. Please keep it within that range.
Former Division: You could have either belonged to one of the 13 divisions in the Gotei 13, the Kidō Corps, or the Onmitsukidō (it is possible to belong to both the Second Division and Onmitsukidō). OCs cannot have held a seat higher than 5th in their history.
Residence: Somewhere in the human world. OCs will be at Hidenseidō or on their own somewhere.
A P P E A R A N C E
Two paragraphs (minimum) are required with no image (if an image if used, only one paragraph is necessary). Make sure to include height, weight, hair color, hair length, eye color, skin tone, facial features, body composition, clothing, etc. Don't include hollow mask appearance.
P E R S O N A L I T Y
Three paragraphs (minimum) are required. Describe things like general attitude, inner emotions, bonding/relationship patterns, outlook on life, faults, habits, strengths, hobbies, likes, dislikes, morals, etc.
B A C K G R O U N D
Five paragraphs (minimum) are required. Make sure to include human life, death, coming to Rukongai, living in Rukongai (if born in Rukongai, your history would start there), passing the Shinigami Academy entrance exam and enrolling, academy life, zanpakuto formation, academy graduation, Shikai attainment and mastery, being infected with hollow reiatsu, turning rogue, life after, etc.

Hollow Mask Appearance: One paragraph or one picture. No canon rip-offs.
Mask Control: OC's begin with beginner control, meaning you cannot draw your mask at will and your inner hollow can come out at random times. Controls: beginner, practitioner, expert, master.
Inner Hollow: One paragraph or picture for appearance and at least one paragraph for personality.
Zanpakuto Name: Usually in Japanese. Provide translations please.
Zanpakuto Spirit: One paragraph of personality as well as a paragraph OR image for physical appearance.
